The IM017 for it wrote that can only completed by a doctor.
How would we proceed?
Yes, I’ve done this a while back, Just an FYI you can send multiple forms in the same envelope.
PS I sent it as registered mail so I got the proof that it was sent.
Do we change the word child to offspring?
There is a correct way to fill it out it is after all a contract cross anything out e.g. it asks to use block letters cross it out put any letters also you should never put child or children also where there is a gap you need to turn it into a z it was left blank intenionaly I all so put all right reserved and put my autograph and stuck the to forms together as to join the meeting of minds make sure you also put square brackets around your post code anything with [ 4444] brackets does not exist on paper

Why do we put square brackets around the postcard if it means it does not exist? Is that because the numbers do not actually relate to our place of residence? Thank you.
Also, what exactly do you mean by put a ‘z’ where there is a gap? Thanks

We have a group that did this letter. You sign it in blue biro only.
(Black pen is dead person)
(Blue pen is business)
(Red pen is blood)
(Purple pen is royalty).
We also have a letter to stop the Govt giving your vaccination information to anyone else.
This letter, you print out 11x of original copies (you can’t use photocopies).
You sign each one in blue pen (never use black pen). I’m happy to share my letters donnapike2450@gmail.com
